Sarah Kolacz, NP

Sarah’s healthcare career began as a nursing assistant on the medical surgical unit at St. Joseph Medical Center in Plymouth, IN. She earned a Bachelor of Science in Nursing from Indiana University South Bend and worked as a registered nurse at St. Jo Mishawaka campus before earning her master’s degree. She then transitioned to her role as a family nurse practitioner at Granger Community Medicine before joining Everside Health. Sarah’s passion is for helping others and uncovering the root issues for disease and illness. She is committed to providing comprehensive and holistic care that’s individualized through her patient partnerships.
